Nurse Case Management

Home-Aide Care Solutions provides Registered Nurses as supervisory case management for all its cases. Our nurses have experience at some of the top teaching hospitals in Boston.  They also have extensive home care backgrounds which compliments their clinical experience from the hospital.

Our nurse case managers will work closely with the caregiver, companion and client to coordinate a personalized schedule and plan to meet each individual’s needs. They have the necessary backgrounds in Alzheimer’s disease and dementia for some of our senior home care clients. We also help our adult clients with various health challenges such as cognitive impairments and oncology treatments.

They provide guidance, assessment, and communication on every case. Having a nurse case manager enables our clients to make more informed decisions and ensures proper care and safety on all cases. We also offer intensified case management for clients with complex clinical or medical needs at an additional fee.
